PLEASE FOLLOW THESE STEPS TO ENROLL YOUR CHILD:

Please download the forms as they are set with pre-fillable spaces. Once completed, please print and sign. All forms are to be filled out on the computer to ensure all information is completed and compatible with sending to our preschool portal Procare and helps with us in creating sign in sheets, class forms, etc. Information on these forms are extremely important. Verify all lines are completed. We will only accept completed forms.

Please include a current photo of your child on the enrollment form.(headshot only) Fees are to be paid at registration to reserve a spot and this is all based on availability. Please bring your Photo ID so you can complete the notarized form with the Director.

The Physician Signed Health Statement & current immunization record is due the week prior to the first day of school. We are unable to accept a student without these forms.

Please note classes are based on availability & staff .
