PLEASE FOLLOW THESE STEPS TO ENROLL YOUR CHILD:
- Download all the forms below.
- Submit all forms below except for the Physician Signed form & current immunization record which are due 1 day before school starts.
- Registration and Supply fee are due at time of registration using our online payment system, ProCare. (No cash or checks are accepted)
- Please note every student must have the Food Allergy Action Plan (FARE) form signed even if no allergy.
- Submitting forms does not give a guaranteed spot in the program as it's based on availability. Once we confirm via email that we have space, we will charge your account via our Procare Online Payment System.
