PLEASE FOLLOW THESE STEPS TO ENROLL YOUR CHILD:

    1. Download all the forms below.
    2. Submit all forms below except for the Physician Signed form & current immunization record which are due 1 day before school starts.
    3. Registration and Supply fee are due at time of registration using our online payment system, ProCare. (No cash or checks are accepted)
    4. Please note every student must have the Food Allergy Action Plan (FARE) form signed even if no allergy.
    5. Submitting forms does not give a guaranteed spot in the program as it's based on availability. Once we confirm via email that we have space, we will charge your account via our Procare Online Payment System.
